Understanding

What is Multiple Sclerosis?

Multiple Sclerosis (MS) is a chronic, autoimmune disease of the central nervous system. In MS, the body's immune system mistakenly attacks myelin, the protective fatty sheath that covers nerve fibers in the brain and spinal cord. This causes inflammation and scarring (plaques), disrupting electrical signals.

Symptoms can vary from temporary numbness to difficulty walking or visual impairments. While there is no cure, today's Disease-Modifying Therapies (DMTs) can suppress immune activity, significantly reducing relapse rates and slowing disability progression.

Types of Multiple Sclerosis

Multiple Sclerosis manifests differently in each patient, following distinct progression patterns.

Relapsing-Remitting MS (RRMS)

The most common form (85% of cases). Defined by clear relapses (attacks) followed by periods of remission where symptoms resolve completely or partially.

Relapse Attacks Remission Phases

Primary Progressive MS (PPMS)

Steady worsening of symptoms from the onset without distinct relapses or remissions. Typically diagnosed in patients after age 40.

Steady Decline No Remission

Secondary Progressive (SPMS)

Follows an initial relapsing-remitting course. Over time, the disease transitions into a progressive phase with steady worsening of symptoms between attacks.

Symptoms & Warning Signs

MS symptoms vary depending on the location of the myelin scarring in the brain, spinal cord, or optic nerves.

Motor & Sensory
  • Limb numbness or weakness
  • Unsteady gait / balance loss
  • Electric shock sensations
  • Muscle spasms or rigidity
Vision & Cognitive
  • Optic neuritis / vision loss
  • Double or blurred vision
  • Brain fog & memory issues
  • Chronic fatigue & dizziness
Heat Sensitivity

Many MS patients experience a temporary worsening of symptoms when their body temperature rises due to exercise, hot baths, or warm weather (Uhthoff's phenomenon).

Diagnosis

Diagnosing Multiple Sclerosis

Since there is no single test for MS, diagnosis requires proving "dissemination in time and space"—lesions in different brain areas occurring at different times.

1

Clinical Evaluation

A detailed history of neurological symptoms, relapses, and physical exams tracking sensory and motor nerve deficits.

2

Brain & Spine MRI

High-resolution scans using contrast dye to detect active and scarred lesions (plaques) within myelin structures.

3

Lumbar Puncture

Analyzing spinal fluid to look for oligoclonal bands or specific immunoglobulins indicative of an active CNS immune response.

4

Evoked Potentials

Testing the speed of electrical signals traveling through visual or sensory nerve pathways to detect myelin slowing.

Comprehensive Multiple Sclerosis Treatment

Although MS cannot be cured completely, modern immunomodulatory treatments and supportive therapies allow patients to lead active lives.

Disease-Modifying Therapies (DMTs)

Targeting the immune system response

DMTs (oral drugs, self-injections, or IV infusions) suppress or modulate the immune system, reducing the number of relapses and slowing lesion formation.

  • Oral, injectable, and monoclonal antibody infusions
  • Slows down accumulation of motor disability

Acute Relapse Management

Rapidly resolve flare-ups

During active flares, high-dose intravenous corticosteroids are administered to quickly reduce nerve inflammation and speed up recovery times.

  • High-dose IV methylprednisolone infusions
  • Plasmapheresis (plasma exchange) for severe flares

Physical Rehabilitation

Rebuilding mobility and balance

Physical and occupational therapies focus on balance drills, coordination training, core strengthening, and adapting activities to fit mobility constraints.

  • Balance, coordination, and gait retraining
  • Occupational adaptations & assistive devices

Symptomatic Management

Relieving fatigue, pain, and spasticity

We prescribe targeted medications to manage chronic fatigue, nerve pain, muscle spasticity, and bladder dysfunction associated with MS.

  • Muscle relaxants (baclofen/tizanidine) for spasms
  • Medications for neuropathy pain and fatigue
